Support at Home Package Coordination

Support at Home Coordination

A coordinated package of care and services to help you to live independently in your own home for as long as you can.

Wellness Centres  

Wellness Centres

Individually tailored strength and balance programs in a safe, inclusive, fun and supported environment increasing your mobility, independence and confidence. Group education sessions are also provided by the health professionals in each region.

Independence Programs

Care Forward specialises in reablement services for people with frailty, chronic illness or following a health setback who wish to make gains in their physical, social and emotional wellbeing.  This may be a single service or an intensive individualised package of supports for up to 6 weeks, focusing on regaining lost functionality and/or learning new skills to ensure that you continue to live at home independently.

Independence Programs

We can help you, whatever your funding source

Depending on your assessed needs and requirements, our services can be provided through a range of different programs, including but not limited to:

Commonwealth Home Support Program (CHSP)

CHSP is for people who require entry level support and services at home. To access this program, you will need to register with My Aged Care for an assessment. 

Tasmanian

HACC Program

This program assists people under 65 years or Aboriginal and Torres Strait Islander people under 50 years who need basic support services to continue to live in the community with dignity. To access this program you will need to register with the Tasmanian Community Referral Service.

Support at Home

Support at Home is a coordinated package of care and services to help you to live independently in your own home for as long as you can. To find out if you are eligible for a Home Care Package, contact My Aged Care 1800 200 422. 

 

You will be referred for an assessment. The assessment will determine if you are eligible for a Support at Home package and which package level best meets your care needs.  â€‹â€‹â€‹

The Restorative Care Pathway

The Restorative Care Pathway focuses on clinical and allied health support. the aim is to keep you independent by building your strengths and capabilities.

 

You can access this pathway while still receiving your ongoing care package. It offers an intensive 12 weeks of restorative support through allied health services.

 

It may be extended to 16 weeks if assessed as beneficial. 

Medicare funded services

Individuals may access allied health services under Medicare if they have a chronic (or terminal) medical condition and a GP has provided a chronic disease management plan.  The GP must provide the referral for the relevant allied health services. The number of Medicare subsidised visits for any particular allied health service is limited to a maximum of five consultation/sessions per calendar year.

That means you can request a referral for any of our allied health and therapy services from your GP.

Private

services

Care Forward offers fee-for-service health and home care services.
You don’t need an assessment, and you can contact us directly to find out what we can offer.

You may be able to claim services through your private health insurer. (HICAPS available for direct billing of health funds)
 

You may be eligible to claim services through MAIB or workers’ compensation insurance.
