There is no obligation to receive care from us.

We're proud of our approach, which is to meet with you, face-to-face, for a conversation.

Our liaison staff want to understand your unique circumstances, your community of care (family and friends and other social connections), and how to get you the support you need to be happy and healthy in your own home.

We'll involve your family and friends as much or as little as you like. We generally encourage you to have them with you when we talk with you.​​​
