Team Leader Clinical and Home Care Package Coordination (Registered Nurse) - North-West Team
Care Forward is an innovative, leading provider of Home Care Package Coordination services across Tasmania, with continued unprecedented growth in Home Care Package client numbers. Our interdisciplinary team specialises in wellness, reablement and restorative care with 21 years of experience in delivering better health outcomes for our clients. Home care package numbers are continuing to grow beyond all expectations and to support ongoing expansion we are recruiting a Registered Nurse to provide leadership for our growing North West team. We have an office in Devonport and are currently establishing a satellite office in Burnie to provide better access for our clients and staff.

About You
You are an enthusiastic and dedicated Professional with a real passion for improving the lives of our clients and supporting a team of health professionals to deliver quality client care and support. With the ability to work autonomously and lead your team, you will thrive in this multidisciplinary environment. You are adaptable, enjoy meeting new people and working in a variety of settings. Additionally, you will possess the following:

Relevant Professional/Tertiary Qualification
-
Registered Nurse with current unrestricted AHPRA registration

Current unrestricted Tasmanian Drivers Licence
-
Excellent communication, time management and leadership skills
-
5 years of experience in a community based or other relevant healthcare setting

Experience in a leadership role
